Questionnaire

Before You Begin

This questionnaire helps me understand your son’s situation in context. There are no right or wrong answers – I’m looking for honest detail about patterns, not judgments about parenting. The more specific you can be, the more useful my analysis will be.

All responses are strictly confidential.

Please note: This guidance is educational and strategic in nature, not therapy or clinical treatment. If your son is experiencing mental health crisis, self-harm ideation, substance abuse, or other serious clinical issues, professional therapeutic intervention is necessary and should be sought immediately.

Please describe his current daily reality: education status, work situation, living arrangements, typical daily structure.
Tell me about the moment or period when you realised something had changed. What was different?
What was it about? What did each of you say? How did it end?
Be as specific as you can: wake time, what he does, when, for how long, who with, etc.
Even a small moment. What happened? What was he doing?
Include online and in-person. How do these friendships function?
Where do you see competence, even if limited?
What tasks, situations, or demands consistently defeat him?
Diagnoses, recommendations, observations.
e.g., eldest, middle, youngest, only child.
Brief sketch of the sibling landscape.
Chores, rent, cooking, cleaning, financial contribution, etc. What’s expected and what actually happens?
Who decides? How much agency does he have? What’s negotiable vs. non-negotiable?
Try to answer from his perspective, not yours.
Be specific. What would you observe that would tell you things are moving in the right direction?
What is he actually good at? What does he value? Where has he shown capability in the past?
tarting/leaving school, moving, legal issues, health concerns, deadlines, etc.
